  • Animal's medical condition and urgency of treatment.
  • Details of the veterinary clinic or hospital.
  • Financial situation and reason for requesting assistance.

Required Documentation Checklist

  • Veterinary estimate or treatment plan from a registered clinic.
  • Medical report or diagnosis from a licensed veterinarian.
  • Proof of financial need (e.g., income statement, NGO registration).
  • Photographs of the animal (if applicable).
  • Identification proof of the applicant (Aadhaar card, NGO certificate, etc.).
